Sympathetic tone climbs
Resting heart rate drifts above your own baseline while short-term HRV flattens — the earliest measurable sign of rising thyrotoxic drive.
ECG early warning · TPP
NuraSesne reads a single-lead ECG and follows the autonomic and electrical shifts that come before a thyrotoxic periodic paralysis episode — heart rate, HRV and QTc — so rising risk reaches you while there is still time to act.

Physiology
Thyroid hormone excess keeps the sympathetic nervous system in overdrive. As an episode approaches, that overdrive intensifies and potassium shifts into cells — changes that surface in heart rate, HRV and the QT interval hours before muscle weakness.
Illustrative trends of expected signal behaviour. The dashed line marks the alert threshold two hours before an episode — not measured patient data.
T − 6 h
Resting heart rate drifts above your own baseline while short-term HRV flattens — the earliest measurable sign of rising thyrotoxic drive.
T − 2 h
As potassium moves into cells, QTc lengthens and T waves flatten. The risk score crosses the threshold set with your clinician.
T − 0
The episode you had two hours to prepare for — with guidance already in front of you and your care circle aware.
Signal path
Every number on this page comes from one sensor and one stream. Six stages, each with a single job.
01 — 06
Stage 01
A single-lead ECG sensor sits on the body and streams the raw electrical signal — P wave, QRS complex, T wave — sampled continuously and sent over Bluetooth or cable. No wearables, no derived step counts: just the heart's own electrical trace.

Stage 02
Raw ECG carries movement noise, 50 Hz mains interference and respiratory drift. A bandpass filter keeps the heart's band, a notch filter removes the power line, and baseline correction flattens the slow swing — leaving a waveform the next stage can trust.

Stage 03
Pan-Tompkins style detection locates the apex of each QRS complex. The gap between consecutive peaks is the RR interval in milliseconds — the raw material for heart rate and every HRV metric that follows.

Stage 04
HR is 60,000 divided by the mean RR interval. From the RR series come SDNN, RMSSD and the LF/HF ratio — a live picture of autonomic balance. From the waveform come QTc, T-wave amplitude and ST behaviour, the electrical signature of a potassium shift.

Stage 05
Each 15-minute window is compared with your own seven-day baseline over a six-hour lookback. An LSTM learns how those trends unfold in time; XGBoost combines the learned pattern with tabular triggers into a single risk score between 0 and 1.

When the score crosses the threshold agreed with your clinician, the alert arrives on screen and as a push notification: the risk, the features driving it, and the preventive steps already written into your care plan.

Personal baseline
A resting heart rate of 86 bpm means something different for every patient. NuraSesne keeps a rolling seven-day baseline and scores your deviation from your own normal — then keeps learning as it settles.

Model
TPP precursors unfold over hours, not seconds. The LSTM holds that history; XGBoost turns it into a decision you and your clinician can inspect.

Risk 0–1
Two-hour probability, thresholded with your clinician
The model reads the slope of heart rate, HRV and QTc — a single odd reading is not enough to raise an alert.
Every alert can show which features pushed the score up, so you and your clinician can judge whether it makes sense.
Thresholds are set per patient and reviewed as your personal baseline settles, rather than fixed for everyone.
The warning
At T−2 hours the monitor shifts from observation to guidance: what the score is, which signals are driving it, and the preventive steps already agreed with your clinician.
Alerts are probabilistic. They support your care plan; they do not replace emergency care or clinical judgement.

Scope
01
A single-lead ECG sees rhythm and repolarisation, not everything. It is one input into your care, not a full cardiac workup.
02
A risk score expresses likelihood. It can rise without an episode and can miss one — your clinician's judgement always comes first.
03
If you cannot move, collapse, or have trouble breathing, contact emergency services. Do not wait for an alert.
